Partner’s Sexual Behavior Assessment

As a betrayed partner, your recovery comes first. You can heal whether or not you are still with your partner and whether or not your partner chooses recovery. This questionnaire assesses your partner’s sexual behavior. It’s valuable information for your therapist to understand the depth of your partner’s addiction. If we are treating you as a couple, it’s good to compare the results of how both you and your partner evaluate his sexual behavior. Please feel free to skip this if you are not with your partner, your partner is rejecting recovery, or if you become too triggered as you fill this out.
